The Awards have, over the past 35 years, established themselves as the benchmark for provider service levels in the industry. Over 4,000 of you voted last year and we'd like to canvass your opinion once more.
This questionnaire will take 5 to 8 minutes to complete and gives you the opportunity to say which providers have given you quality service throughout the past year - and which ones haven't.
Please read the instructions and definitions before filling in the questionnaire and remember to only rate those companies you have done business with in the last 12 months.
We can only accept completed questionnaires from FCA-authorised intermediaries and mortgage advisers who have completed and submitted the survey themselves.
All other entries will be disqualified. The information you give will be reported anonymously in an aggregated format and you will not be identified personally.
